Dog Information Form

Required annually for each participating dog attending Vickery K9 services, including private lessons, group classes, workshops, and board & train programs.

If you have already completed this form during the current calendar year for the dog you registered for a class or appointment, and your information has not changed, you may disregard the instructions below.

To complete the Dog Information Form:

1. Click here.

2. Select any available time.

3. Click “Select & Continue” to be directed to the registration form.

4. Complete the form and click “Confirm Appointment” to submit your information.

Please note: Our scheduling system requires forms to be submitted through an appointment-style format. Selecting a time does not schedule an additional class or appointment.

If you are registering multiple dogs, please complete a separate form for each dog.

Required for private training clients prior to their first session.

Group class clients complete applicable policies and agreements during registration and do not need to complete this separate form unless otherwise instructed.
